ServePA Vaccination Clinic

MUST COMPLETE REGISTRATION EXTERNALLY FROM VOLUNTEER.PITT.EDU.
FOLLOW THE INSTRUCTIONS BELOW:

1. Register with SERVPA
        1) Go to the SERVPA home page: https://www.serv.pa.gov
        2) Click on the registration link.
        3) Under “Organization” click “Add Organizations.” This is where you can sign up as a volunteer for the mass vaccination clinics.
                  a) Select “COVID Support” then click select organization. Click “COVID Mass Vaccination (PADOH); Click on the + and select “University of Pittsburgh School of Pharmacy” from the drop- down selections
                  b) If you also would like to hear about other local volunteer opportunities, please register for the Allegheny County Medical Reserve Corps by selecting “Medical Reserve Corps (MRC) Organizations”; Click select organization. Check the box next to the desired county (Allegheny for Pittsburgh clinics).
        4) Under “Occupation type” select “non-medical.”
                  a) Under “Occupation” select the appropriate choice.  There is an “other” option available. 
        5) Fill in any other remaining required information. Please complete your profile to 100%.

2. Complete the PITT Mass Vaccination Volunteer Form: New Form is being created and will be forwarded ahead of the event.

3. Complete the Allegheny County Department of Health Indemnification/Maintenance of Security and Privacy of PHI Agreement via this link:  https://pi.tt/release
 
In addition, all in-person volunteers will need to provide Date of Birth in addition to their full names as to run their information through the YMCA’s child abuse clearance system.
